Findings: Alleged Perpetrator #2 (AP2) and Alleged Perpetrator #3 (AP3) failed to follow the Alleged Victim's (AV) care plan to assist him/her to get up for the morning and to breakfast. On or about March 7/ 2020, AV was in his/her room and fell from his/her chair sustaining an injury to his/her head requiring transportation to the hospital. AP2 and AP3's actions are considered neglect of care and constitute abuse. The facility failed to provide a safe environment, which is a violation of Oregon Administrative Rules.
