Findings: The facility failed to ensure supervision and staff support regarding known fall risks related to the Alleged Victim (AV). On or about March 6, 2020, AV had a injury fall when he/she stood up from his/her recliner and fell. Staff could not reach AV in time to keep him/her from falling. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
