Findings: The facility failed to assess and intervene when Alleged Victim (AV) suffered from a fall on February 11, 2020. After the fall on February 11, 2020, the facility failed to put interventions in place to prevent AV from falling again. On March 4, 2020, AV suffered from another fall and was sent to the hospital for treatment. The facility failed to properly care plan for AV's fall risk which is a violation of resident rights, is considered neglect of care and constitutes abuse.
