Findings: Witness 1 (W1) is care planned to be separated from other residents due to his/her history of grabbing onto other residents resulting in injuries. On or about March 15, 2020, W1 and Alleged Victim (AV) had an altercation that resulted in AV getting bruises. The facility failed to protect AV from W1's aggressive behaviors which is a violation of resident rights, is considered neglect of care, and constitutes abuse.
