Findings: Witness 1 (W1) has a known history of physical and verbal aggressive behaviors. On or about March 17, 2020, W1 entered Alleged Victim's (AV's) room and began yelling at AV. W1 grabbed AV's wrist and violently shook AV. The facility failed to provide a safe environment which is a violation of resident rights, is neglect of care and constitutes abuse.
