Findings: The facility failed to appropriately care plan regarding Alleged Victim's (AV) falls to ensure safety and prevention of future falls. AV had a fall on February 11, 2020 and was sent to hospital for treatment. AV returned to the facility on March 2, 2020 after a short stay at a skilled nursing facility. AV fell again on March 4, 2020 and on March 22, 2020; both falls occurred when AV attempted to transfer on his/her own. An investigation determined that facility did not add any fall preventions and/or interventions to prevent AV from future falls. The facility's failure to properly plan care for AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
