Findings: On or about January 28, 2020, Alleged Victim fell while trying to transfer on his/her own resulting in a head injury that required AV getting six (6) staples on his/her head. An investigation determined that AV had four falls between November 21, 2019 and January 28, 2020 with two of the four falls causing injury to AV. The facility failed to properly care plan for AV's fall risk which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP21-02439 $1125.00 fine assessed
