- On or about March 27, 2020, Alleged Perpetrator 2 (AP2) touched Alleged Victim (AV) inappropriately and in a sexual manner while AP2 was acting in the role as AV's caregiver and while attending to AV's care needs. AP2’s is responsible for sexual abuse. The facility failed to protect AV from sexual abuse which is a violation or Oregon Administrative Rules.
