Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about March 25, 2020, AV was discharged from the skilled nurse and the facility was provided with AV’s discharge date. The facility accepted AV back but did not address AV’s change of condition at re-admit. On or about March 28, 2020, staff head a noise from outside AV’s room, staff entered the room and found AV had fallen to the floor. AV was transported to the hospital and found to have a fractured hip. The facility failed to properly re-admit AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP21-02802 $450.00 fine assessed
