Findings: Alleged Victim (AV) was diagnosed with mild cognitive impairment. Facility staff were aware that AV had been demonstrating increased confusion, increased resident to resident altercations and increased falls. On or about March 28, 2020, AV had three unwitnessed falls in his/her room sustaining injuries. AV was transported to the hospital for treatment. The facility failed to respond appropriately to AV's change of condition putting AV at risk for serious harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP20-00596 $500.00 fine assessed
