Findings: The facility failed to appropriately care plan related to the Alleged Victim's fall history. The facility had inappropriate interventions, placing the Alleged Victim at a risk of harm from falls. The Alleged Victim had multiple falls from January 2020 through March 2020 without appropriate change to his/her care plan. The facility's failures are a violation of resident rights, are considered neglect of care and constitute abuse.
