Findings: Amended 4/23/2021: On or about March 28, 2020, Alleged Victim (AV) and Witness 1 (W1) had a resident-to-resident altercation. Prior to the incident, W1 was placed on a visual/indirect one-on-one care plan; however, an investigation determined facility staff were inconsistent in their knowledge of W1's one-on-one care needs. The facility failed to ensure W1 received one-on-care which resulted in the altercation between W1 and AV. The facility's failure is a violation of Oregon Administrative Rules.
