Findings: The Alleged Victim (AV) and Witness #1 had prior altercations and had an unwitnessed altercation on or about April 1, 2020. W1 was to be redirected from AV and AV's doors was to be locked to prevent altercations. AV suffered at bruised knee after the incident. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
