Findings: Witness 1 (W1) had a known history of inappropriate sexualized behaviors towards Alleged Victim (AV) and other residents. W1 engaged in sexualized behavior approxemetly ten times prior to this incident, with four of them being with AV. On or about April 5, 2020, W1 entered AV's cottage, approached AV and began rubbing and grabbing AV's breast. The facility failed to care plan and implement appropriate interventions to prevent W1 from victimizing AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
