Findings: The facility failed to provide a safe medication administration system to ensure Alleged Victim's (AV) physician orders were followed. On or about March 24, 2020, AV's physician order for his/her seizure medication dosage was increased and the facility did not catch this change until April 8, 2020. The investigation also identified that Alleged Perpetrator 2 (AP2) failed to follow AV's MAR on April 7, 2020 and again on April 8, 2020. AV suffered from increased tremors as well as experienced a decrease in his/her ability to manage his/her ADLS as a result of AV's medications not being administered as ordered. AP2's actions is considered neglect of care which constitutes abuse. The facility did not provide a safe medication administration system for AV which is a violation of resident rights, is considered neglect of care and constitutes abuse.
