Findings: Alleged Victim (AV) has a diagnosis that effect cognition and memory. AV has accompanying anxiety, agitation, and behaviors. On or about April 9, 2020, AV was agitated, trying to get up, and calling out. Alleged Perpetrator 2 (AP2) was trying to assist AV with standing up and AV called AP2 a derogatory name. AP2 responded using profanity and told AV to shut up and threatened physical harm. AP2's actions are considered verbal abuse. The facility failed to protect AV from verbal abuse which is a violation of Oregon Administrative Rules.
