Findings: Witness 1 (W1) and Alleged Victim (AV) have a known history of resident to resident altercations. On or about March 20, 2020 and April 15, 2020, W1 and AV got into an altercation in the kitchen/dining area. The facility failed to provide a safe environment putting AV at risk for serious harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
