Findings: The facility failed to provide a safe environment for Alleged Victim (AV) by failing to ensure the safety of AV when AV is wandering around the facility. On or about April 11, 2020, AV and Witness 1 (W1) had an altercation that resulted in W1 hitting AV which caused AV to fall to the floor and sustain a scratch on his/her ear. At time of incident, AV wandered into W1's bedroom. The facility failed to keep an eye on AV and to redirect AV if he/she is wandering into another resident's room. The facility's failure to provide a safe environment for AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
