Findings: The Alleged Victim (AV) was care planned to transferred with two staff and a sit to stand. On or about April 3, 2020, Alleged Perpetrator #2 (AP2) transferred AV to the restroom without the sit to stand. AV began to fall and AP2 assisted AV to the ground. AV was not injured, however, AV was placed at risk for harm. The facility's failure to follow the care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP23-00204 $188.00 fine assessed
