Findings: The facility failed to provide a safe environment for Alleged Victim (AV). On or about April 16, 2020, AV fell while in the bathroom and suffered a bump to his/her head along with an injury to his/her right arm. An investigation determined that AV's fall was most likely caused by slipping on the wet floor of the bathroom. The facility did not provide AV with a safe environment by allowing water to be on the bathroom floor and by not monitoring AV adequately. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP20-01054 $188.00 fine assessed
