Findings: Alleged Victim (AV) requires assistance with medication administration. On or about April 23, 2020, Alleged Perpetrator 2 (AP2) administered 80 units of fast acting insulin instead of 80 units of slow acting insulin. AV was transported to the hospital for observation. At the time of the incident the facility was understaffed and AP2 was working a double shift. AV's insulin pens were stored in the same container. AP2's actions are considered neglect of care and constitutes abuse. The facility failed to provide a safe medication administration system, which is a violation of resident rights, is neglect of care and constitutes abuse. **As of August 11, 2020, this facility is closed and therefore will not receive a civil penalty.
