Findings: On or about April 28, 2020, Alleged Victim (AV) and Witness 1 (W1) had a resident-to-resident altercation that resulted in AV being injured when W1 pushed AV to the floor. An investigation determined the facility did not follow AV's and W1's care plan which placed AV at serious risk of harm The facility's failure to follow AV's and W1's care plan is a violation of resident rights, is considered neglect of care, and constitutes abuse.
