- Findings
- The facility failed to provide a safe environment for the Alleged Victim (AV) or to implement adequate interventions to prevent his/her falls. AV had multiple falls between March 28, 2020 and May 2, 2020, resulting in injuries from a bruise to a fractured pelvis. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
