Findings: Alleged Victim (AV) is a known fall risk. From March 22, 2020 through May 4, 2020, AV experienced five (5) falls in his/her room from attempts to self-transfer. AV's service plan states that AV is confused and not able to use the call light. Interventions include reminding AV to use his/her call light and urinal. The facility failed to care plan and implement appropriate interventions based off AV's abilities to mitigate AV's risk of injury due to self transferring, which is a violation of resident rights, is neglect of care and constitutes abuse.
