Findings: The facility documented that Alleged Victim (AV) suffered from skin breakdown on or about February 25, 2020. AV experienced a rapid decline and started refusing care from staff sometime in March 2020. AV received wound care from an outside agency from approximately March 4, 2020 through May 1, 2020. On or about May 4, 2020, AV went to the hospital and it was noted that the skin breakdown present did not appear to be taken care of by the facility. AV's care plan did not address AV's increase in refusal of care and how staff are to approach AV. The facility failed to care plan for AV's change of condition, which is a violation of resident rights, is neglect of care and constitutes abuse.
