Findings: The Alleged Victim (AV) was care planned to have cares completed by two staff; however, on or about May 4, 2020, Alleged Perpetrator #2 (AP2) failed to call a second caregiver while responding to a call light engaged by AV. AV experienced unreasonable discomfort when AP2 responded alone, had behaviors towards AP2 and had to have other staff restrain AV to avoid harm to AV or others. AP2's actions are considered neglect of care and constitute abuse. The facility failed to ensure care plans were followed which is a violation of Oregon Administrative Rules.
