Findings: On May 8, 2020 and May 9, 2020, the facility failed to follow Alleged Victim's (AV) care plan during meal team when the facility staff allowed Witness 1 (W1), a resident of the facility, to force fed water to AV. W1 was observed being physical during both incidents. The facility's failure is violation of resident rights, is considered neglect of care which constitutes abuse.
