- Findings
- Alleged Perpetrator 2 (AP2) failed to follow Alleged Victim's care plan when AP2 assisted AV with his/her shower. As a result, AV became agitated which caused AV's anxiety and emotional behaviors to escalate. AP2 was observed slapping AV on the face and pulling AV's hair, causing AV's head to pull to the side. AP2's actions is considered physical abuse. The facility failed to protect Resident #1 from inappropriate verbal comments made by staff and the failure is a violation or Oregon Administrative Rules.
