Findings: The facility failed to provide a safe environment and ensure all necessary furniture was appropriately fastened to the wall to prevent injury. The failure resulted in the Alleged Victim (AV) being discovered with his/her wardrobe on top of his/herself and was transferred to the hospital where s/he passed away from injuries, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
