Findings: The facility failed to follow the Alleged Victim's (AV) care plan to provide supervision to keep him/her safe. The failure resulted in AV wandering into another residents room unsupervised and was found wearing only a T-shirt and briefs, which exposed AV to harm and resulted in a loss of personal dignity, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
