Findings: The facility failed to provide a safe medication administration system to ensure the Alleged Victim (AV) received his/her medication in a timely manner. On or about May 11, 2020, AV had medication that was delivered to the facility, however, AV never received his/her medication and it could not be found in the facility. Alleged Perpetrator #2 was working the day the medication arrived, however, there is no signature or documentation that he/she signed for the medication. The facility's failure to process incoming medication properly placed the AV at a risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
