Findings: The facility failed to ensure the Alleged Victim's safety due to a lack of service planning for his/her history of behaviors. The AV suffered a bruise during care due to him/her having behaviors and attempting to hit staff and hit his/her face on the bed rail causing the bruise to his/her cheek. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
