Findings: The facility failed to provide staff that was properly trained to transfer the Alleged Victim (AV) during ADL's. On or about May 20, 2020, Witness #2 (W2) was transferring AV during toileting assistance where AV fell, sustaining a bruise. W2 was not properly trained in transfers prior to giving this assistance to AV. The facility's failure to properly train staff is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP20-01241 $188.00 fine assessed
