Findings: On or about May 28, 2020, Alleged Victim (AV) discovered his/her wallet was missing. It is unknown if the wallet was taken by an unknown facility staff member or lost. The facility failed to provide AV with a lockable storage unit to safe guard his/her valuables, which is a violation of resident rights, is neglect of care and constitutes abuse. **The facility closed August 11, 2020 and therefore will not receive a Civil Penalty.**
