Findings: Witness 1 (W1) had a known history of a drinking problem at which time he/she becomes verbally aggressive. On or about May 27, 2020, Alleged Victim (AV) reported that W1 threatened AV with harm. At the time of the incident W1 was intoxicated. The facility failed to care plan according to W1's history which is a violation of resident rights, is neglect of care and constitutes abuse. **The facility closed August 11, 2020 and therefore will not receive a Civil Penalty.**
