Findings: The facility failed to provide a safe environment for Alleged Victim (AV) by failing to protect AV from repeated falls resulting in risk of serious harm to AV. The facility failed to protect AV from falls by neglecting to implement interventions in a timely manner. Between February 18, 2020 and April 28, 2020, AV suffered from at least 8 falls without appropriate interventions. Facility records show that appropriate interventions did not begin until April 28, 2020. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
