Findings: Alleged Victim (AV) requires assistance from staff for all transfers and ambulation. Staff are to put AV's shoes and socks on until AV is in the shower chair. On or about May 27, 2020, Alleged Perpetrator 2 (AP2), transferred AV to his/her shower chair. AV stood up and fell causing AV unreasonable discomfort. AP2 did not put on AV's shoes and sock prior to the transfer. AP2 failed to follow the care plan which is considered neglect of care and constitutes abuse. The facility failed to ensure AV's care plan was followed which is a violation of Oregon Administrative Rules.
