Findings: The facility and Alleged Perpetrator 2 failed to provide the Alleged Victim with necessary care and services when she/he needed to be toileted. The failure resulted in AV experiencing an unwitnessed fall resulting in a neck fracture and requiring surgical intervention, which is a violation of resident rights is considered neglect of care and constitutes abuse.
