Findings: Alleged Victim (AV) is independent with mobility, wanders the facility, and has a known history of exit seeking behaviors and elopement. On or about May 29, 2020, AV passed through an open kitchen door which was not operating correctly and did not self latch. AV then went out to the courtyard and eloped through the courtyard gate which was not properly latched. Following the elopement, facility actions included monitoring AV for exit seeking and having the gate latching mechanism on the courtyard gate fixed. On or about August 18, 2020, AV eloped through the courtyard gate after a maintenance staff used the gate prior. The facility failed to provide a safe environments putting AV as risk for serious harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
