Findings: The facility failed to provide a safe environment for Alleged Victim (AV) which resulted in AV suffering from two falls within a three (3) day time period and for both falls, AV was transported to the hospital for treatment. On or about May 8, 2020, AV was transported to the hospital after suffering from a fall and was diagnosed with a left elbow fracture. On or about May 10, 2020, AV fell a second time, was transported to the hospital for treatment five hours after the fall and was diagnosed with a fractured femer. The facility's failure to protect AV from falls is a violation of resident rights, is considered neglect of care and constitutes abuse.
