Findings: The facility failed to appropriately plan care for Alleged Victim's (AV) risk for falls which resulted in AV falling and getting an injury to his/her face on or about May 28, 2020. AV is a high fall risk and prior to day of incident, AV had previous falls on April 4, 2020, April 7, 2020, and May 24, 2020. On May 28, 2020, AV suffered from a fall while using the restroom without staff assistance. The facility's failure put interventions in place to prevent AV from falling is a violation of resident rights, is considered neglect of care and constitutes abuse.
