Findings: Alleged Victim (AV) had a known history of falls. AV had falls on May 2, May 8, May 14, May 20 and two on May 25, 2020. After each fall the temporary service plans called for increased supervision and safety checks. AV was not supervised between the hours of 10:30pm and 12:40am on May 30, 2020, and was found outside alone in the courtyard with injuries due to falling. The facility failed to follow the care plan, which is a violation of resident rights, is neglect of care and constitutes abuse. **On August 11, 2020 the facility closed, therefor a civil penalty will not be issued.**
