Findings: Alleged Victim (AV) was care planned to have staff ensure his/her alarm pads on the bed and chair are on as the AV has a history of unplugging them. On or about June 3, 2020, AV fell in his/her room around 5:00am sustaining a fractured wrist and finger. Staff reported the alarms were in place but not on. the facility failed to follow AV's care plan, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP20-00994 $375.00 fine assessed
