Findings: Alleged Perpetrator 2 (AP2) treated the Alleged Victim inappropriately by attempting to hit the Alleged Victim's hand away, but was intercepted by another caregiver prior to actually hitting him/her, however, staff note that they have seen AP2 hit the Alleged Victim in the past and have reported the incidents to the facility administration. AP2's actions are considered physical abuse, which is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failed to protect the Alleged Victim from physical abuse by not taking measures to ensure the safety of residents by not taking earlier actions regarding concerns brought to their attention by other staff regarding AP2, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
