Findings: On or about June 4, 2020, the Alleged Victim (AV) used his/her call light and summoned for staff assistance. Alleged Perpetrator 2 (AP2)answered the call light and cleared it and advised AV either AP2 or someone would come help him/her. No one responded and AV was left in his/her own feces for an undetermined amount of time, placing AV at a risk for harm. AP2's actions are considered neglect of care which constitutes abuse. The facility failed to provide a safe environment which violates Oregon Administrative Rules.
