Findings: On or about June 5, 2020, the Alleged Victim (AV) reported that Alleged Perpetrator 2 (AP2) had sexual relations with him/her. The facility failed to provide a safe environment for AV by not placing AP2 on leave as soon as the accusation was made, placing AV at a threat of harm. AP2 was found not substantiated for abuse. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
