- Findings
- Alleged Perpetrator 2 (AP2) failed to follow Alleged Victim's (AV) care plan regarding assistance with mobility/transfer. As a result, AV did not transfer well which resulted in AV screaming out in pain and getting a skin tear on his/her calf. AP2's actions is considered neglect of care and constitutes abuse. The facility failed to provide a safe environment for AV which is a violation of Oregon Administrative Rules.
