Findings: The facility failed to appropriately care plan for Alleged Victim (AV) regarding his/her risk of falls. The facility failed to implement new fall interventions after AV's falls on May 24, 2020, May 28, 2020, and June 4, 2020. On June 8, 2020, AV suffered from another fall and was sent to the hospital for treatment. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
