Findings: On or about June 9, 2020, the facility reported it had completed an audit of Alleged Victims (AV's) personal incidental funds account (PIF) from 2017 through 2020. Through the investigative process it was determined that the facility had procedures and processes in place to manage PIF accounts appropriately however these procedures and processes were not always followed causing approximately $3.35 to be unaccounted for. An unknown staff member is responsible for the errors which is considered financial exploitation and constitutes abuse. The facility failed to protect AV from financial exploitation which is considered neglect of care and constitutes abuse. **The facility closed August 11, 2020 and therefore will not receive a Civil Penalty.**
