Findings: The facility failed to timely and adequately assess the Alleged Victim (AV) after he/she suffered a fall and received a goose egg bump on the back of his/her head. By not evaluating the resident in a timely manner, the facility placed AV at risk for serious harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
